Transfer script rights

Hello,

I wrote a 100-page screenplay where I as an individual own all rights. I need to create a form or document that would transfer those rights to my new studio.

Does anyone have a template kicking around?

Why do you need to transfer the rights? I take it you're the owner of both? Do you just want to register it with the WGA under the studio name? Is the studio going to produce the film? If so, then perhaps it is more of an option deal than a rights transfer.

I own the company only in part meaning I'm not the only shareholder. Yes, the company will produce the script into a movie.
