Production Log for P2

Hey All,

I was curious if anyone my be able to share there Production Log form they use on a set when shooting with P2. I will be doing shot next weekend and have devised a few things, but wanted to get the best form possible from those more experienced in P2 since I have only been using it for a few months.

I am used to recording on paper timecodes, shot numbers, tape numbers and maybe comments and such, but P2 adds a lot more. My biggest issue is when shooting and the shot your shooting spils over to your next card, what is the best way to know when this happens and how to record it on paper.

Any suggestions would be awesome,

jeez, no body could maybe share their p2 log, maybe even send me a link or upload it on the forum?

If not, its cool, I will get cracken hard core on what I think will be best, just thought anyone's suggestion would help me out.
