I'm almost positive that to output 4K from the GH4, you will need the breakout box accessory - ie, the camera on-board HDMI port is 1080 max.

(Edit: When I say output 4K, i'm referring to output via cable to external storage. Not output to the camera's own internal SD card.)

No. I is a hardware limitation of the Odyssey7Q. Over HDMI we do not accept 1080 60p or 4K.

On 4K and 1080 4:2:2 - what's the limit when using SDI? (I checked your site, it appears at least 60fps).

In theory we could go well beyond 60p, but no one sends it.
