P2 woes with unrecognized and duplicate files.

When trying to import into premier the third clip is duplicated instead of importing the remaining video files (pic 1).
In P2CMS it only recognizes the first 3 clips (pic 2).
All of the files are there and when previewing with the space bar I can see or hear them (pic 3).
The missing clips will open in mxf quickstat (pic 4).

That's about all the info I can think of providing.
Is there something missing?

...and one other thing is that the clips will show up in P2CMS for just a second with the pink "!" in the corner and then promptly disappear (pic 5). It's my understanding that the "!" means that it is a spanned clip, but none of the clips were spanned. I only had the one card.
