I don't think they were in Spain at the very end. The hospital he was in could have been any where and she could have transported him to any where while he was blacked-out. Also, her plans were to drive to the tip of South America. So I think they were maybe in Central America.
There was a sign in the last shot but I couldn't read what it said. If it said "Spain" then forget everything I just said.
Yes. Come to think of it, I remember one of them saying something about getting to "Cape Horn" which is at the southern most tip of South America. It was an obvious line used to clue us in as to where they were. I got Cape Horn confused with Cape of Good Hope.
So, you're right. He blacked out and she probably transported him to the Americas.
