I know the GH5 AF is not great, but what's the general consensus for the settings to get the best continuous video AF performance? Is it just to leave the custom settings off and use 1-area AF?

I'm still using 1 area AF when needed but still wanting to experiment since the 2.0 firmware update just haven't had a chance yet.

Here's the best combination I've found to date. Note: 'Best' is this case simply means 'least bad'. It's still not going to be perfect.

1) Turn Continuous AF ON
2) AF Custom Settings(Video) ON - AF Speed +5, AF Sensitivity -3 (These settings are very important)
3) Set AFC to Tracking
4) Use the touch screen to set the position of the tracker.
5) Go!

Once you're rolling, you can touch the screen to update the position of the tracker as needed. 1-area AF also works well with these settings, but I prefer the tracker because it essentially combines the functionality of 1-area with the ability to track. Still, both are viable options. But in both cases, use the AF Custom Settings above.

Note: The problem with 1-area is the size of the square plays a big part in how successful or not it will be. And of course the size of the square will be determined by how large or small the object is you're trying to keep in focus. And if the object moves out of the square, you're in trouble. These issues go away if you use the tracker.
