GH2 Flicker and Banding..

I have seen some of my own more recent footage have this horrible Flicker issue like this video https://vimeo.com/23642799
I am not sure what is causing it. I have seen it in my own footage hacked GH2, three lights for an interview, and a white backdrop.
What would be causing this?
Is this related to banding?

that is shutter speed and lighting frequency flicker, usually caused by not matching frequency to shutter speed/fps. 1/50th for PAL 50hz and 1/60th for NTSC 60Hz is the general rule of thumb, there will be times where you will get odd frequencies though.

I switch between 1/40 and 1/50 to get rid of flickering when I see it
 
I had a very important corporate shoot that I used the GH2 on. And luckily I knew to change shutter speed for the fluorescent lighting. I ran it at 60 and it came out fantastic! I had a couple of lights I brought but still had to have the overhead lights on.
