Canon 5D MarkII is grainy!!! Why???

I use my 5D for interviews. The subject is lit 3 point and I add light to the background to highlight different areas. I understand camera settings very well but my video is super grainy even at 200iso.

Im shooting 1920x1080, 30p, shutter at 60, iso at 200 and aperture at 4.0.

Lens: 24-105(4.0) & 50mm(1.8)

Does anyone have any ideas????
 
I haven't shot video on a 5D in over 2 years, but from what I recall you'll get the best results from sticking to the native ISO's, 160, 320, 640, 1250, 2500.
