C100: 70-200 with Canon EF 2.0X III Extender - Blurry (Pic Attached)

I recently shot a wedding where I had to use my EF Extender, and I double-triple checked my focus multiple times throughout the ceremony and for some reason it is horribly blurry. I was using the built in ND's, and the extender.
I've attached a picture where it shows how it's very blurry, and it's not out of focus blur, it almost makes my eyes hurt looking at this still as the blur is just so odd.

Any ideas? Thank you.

Well I'm assuming your shooting 24p at 1/48th of a second using the 200 with the 2x extender giving you a 400mm. Not a very easy combo to shoot at 1/48th if your tripod is weak and forget handheld. Even wind will make a blurry photo/video with that shutter/focal length combo. If this is a static cam without movement, sandbag everything. Tripod, camera, and even weigh down the lens (but use a lens support).
And, you are using an extender. Make sure that version of extender is compatible with the lens. Also, extenders will reduce lens performance.

Maybe mild chromatic aberration? Does look like the focus point is maybe just behind your subjects a bit?

Did you black balance before this shot? What ISO was this shot at and what ISO were you using before this shot?

Which 70-200 lens? Canon or third party?

I think you missed the focus it looks like focus is on the pillars behind the couple. I find my 2nd shooter miss focus a lot lately. I want my 2nd shooter to start filming at F/5-7 to compensate for bad focus, it's a problem.

I used the same lens/extender combination over the weekend with ND filters on, and had much sharper results:

You either have a bad extender or missed focus, I think.

Shot was steady on a tripod, I suppose it's possible I missed focus but I had peaking on and their faces were outlined as they should be. Perhaps i'm relying on peaking too much now that I have it.

It's a Canon 70-200 w/IS, I may have had IS on to compensate for any wind i'm not sure.

Image stabilization can actually blur your image when the camera is mounted on a tripod because it's trying to correct a steady shot and may just be correcting it's own vibrations. Usually, the IS makes an even bigger racket when tripod mounted that you can hear from a few feet away.
