Heart attack? The vieins are visually huge, in order to supply the blood flow to the muscle mass, else they would cramp up. So the heart must be going nuts to keep the flow of blood up. So, it's probably grossly oversized also.

The guy is a bodybuilder. I forget his name but he is not that big. This is P.shop. A person this size couldn't even move well and would not get judged well at an event. It just wouldn't go over well.

It is possible. I saw a guy on Inside Edition who used some kind of drug that he injected directly into his muscles that made his biceps look like balloons. I guess in theory you could inject this stuff in multiple muscles and look like that guy. But I do believe that is photoshop.
