Whirlwind Medusa 12 - Output Mult Box (Press Box)
Asking $250 + S&H
Add $20 if you want the case too (I'll add foam to keep if from being loose in the case)
I'm unsure of the model, but it is close to the Whirlwind PB12

This unit takes a single mic level input & splits it to 12 mic level outputs. Very rugged. It has lost one of the rubber shoes on the bottom. All of the outputs work nice, but many have seen better days (check photos).
A few of them will have some noise when you attach or wiggle a XLR cable, but I haven't found this to be a problem b/c you're usually not pulling on the cable while you're using it.
