What is this Godawful noise?!

Sometimes when I'm editing, i'll be playing something back, then out of nowhere an extremely loud popping and screeching noise happens that scares the crap out of me. There's no warning, and no limiter on how loud it gets. I know that this happens to other people, as I've talked to them about it. But what causes this, and what can I do to stop it from happening? Twice this month my ears have been destroyed by this menace.

All I know is when I edit with headphones and it happens the ear destroying menace is much worse.

lot of threads about this on cow and apple forums
I was getting it too - kept my software updated and it went away

Yeah, I've been reading stuff on the cow, but none of them seem to lock in what's doing it. My software is as up-to-date as it'll ever be (Studio 3).

for what its worth, I'm running fcp7 on osx 10.6.8 with a 2011 macbook pro and have not had it happen yet with this machine
I used to get it the year before on my old iMac

My iMac (2010) does it at home, and my MacPro (2009) does it at work. One is FCS2, one is FCS3, both are on 10.6.8.

When I used to edit on a MBP, it never happened. Hmm... Peculiar.

I have had it happen numerous times and it is pretty much the worst.
iMac
FCP 7
It happened with 5d footage that was given to me. I can't remember how it was transcoded though. I looked for solutions for a while and updated stuff and it went away I believe.

What sort of audio sources are we talking about? Generally that indicates re-sampling.

Used to get this issue from time to time on my mac pro 2,1 (QUad core) never had it on the 4,1 (8 core) - never could work out why. Would just jump out suddenly and randomly, but very rarely.
