Tape problems!!

Since I bought my DVX100A camera several years ago it has produced far more dropouts than my previous DVX100. For example in the past two months I have shot weddings where I get the ventian blind style drop out where the image seperates for several seconds. Last weekend I was shooting a wedding and I got the red circle with an X warning and then and error for "S-lock". I opened up the tape mechanism and the tape had been pulled from the cassette and there were crinkles on the tape.

I had the heads professionally cleaned/checked a month ago. Could this be a tape problem (I use exculsively FijiFilm tapes) or should I have my heads replaced?

I need advise about how to proceed.
 
You probably need to get the heads aligned. I had the same problem with my VX1000, I took it to the service centre, they aligned the heads and everything was fine.
 
also, do you reuse tapes?? i am a professional wedding videographer and never re-use customer's tapes...
