I recently found two of these plates in storage.
One is definitely from a FSB4 head and I believe the other one is from one of the ACE models.

I don't remember if they are universal, so just being accurate with the details. (If they are, it doesn't matter.)
The one to the left (ACE) has a few particles of dust on it and is brand new, and the one to the right (FSB4) is missing all rubber parts on top and is banged up.
$100 for both, shipped. (I have the 1/4-20 screws too.)
