Renting equipment question!

I agreed to rent out my HMC150 to a photographer I know for a day of wedding shooting for $100. Is there any sort of paperwork i should have him sign, liability etc. to protect me and my equipment? If anyone has some sort of contract or something they'd advise using that would be great.
 
1) $100 is really cheap, I think the local place charges $250 for 1 day rental of the HMC-150.

2) YES. You really need to have paperwork done. I think most rental shops require either that you have insurance that covers rented equipment or that you pay an additional fee so you can use their insurance. I think that fee is around $75 for my local shop.
 
well i know the guy personally, he's shot a few photos of us and used to give me some pointers on photos, and he needed a camera for 1 day for $100 and i figured i'd help him out. The camera is insured and i'm sure the odds of it breaking during a wedding is pretty low but regardless.

Does anyone offhand happen to have an already typed contract?
