I was wondering what release forms you use? Permission from Actors, xtras, locations etc. I am filming allot and dont want to get screwed in the end by some form. Thanks
I basically just made one, put my company logo at the top and had a bunch of models sign them. The one I created in a nutshell says. You are giving the producers of my company the right to film you, photograph you and use your voice if interviewed for use in whatever project I am shooting (I am working on producing volume 2 of a DVD I have out now so the title of the DVD is listed on the form as well.) Create your form in a way that they can't come back at you down the road and say you didnt have permission to film or use their picture or voice. Also on mine, I state that I will only use their photographs, video, or voice for that particular project only however they are giving me the right to use them in promotion such as commercials or trailers.
