I am confused about how I should go about sending in the correct rebate information for my DVX100B manufactures rebate.
I've got myself tangled in a confusing web.
See, I work for my church and made a deal out with them that if they bought my DVX100B for me, I would go without pay for the next 10 months, to equal out the total amount they spent. They agreed.
Our church has multiple credit cards. I used the one for the youth department which is under a Lynn Coulter. I set shipping address to my house, and billing address to a a church address under a David Rice, the man in control of funds at our church, and his name is also on the email invoice from the site I bought it.
Now... HOW TO FILL OUT THE REBATE FORM. It calls for personal information like name, address, phone number, etc. Who's information do I fill out? Do I put David Rice as name then my own address? Do I just put only my personal information in the rebate form? What? Maybe someone else has been in this situation. I don't know if the rebate from assumes the shipping and billing address match.
Suggestions are greatly appreciated, I'm coming up on my rebate's deadline, so I need this sent in soon. Please help!
