what am I missing?

Catering?
Walkie-talkies?
Transportation?
Plenty of tapes and batteries?

-Or-

Director's Chair and old Baseball Cap?? Hehe

location(s) secured, transportation, extra cash for the unexpected..., shoot the rehearsals ( don't forget the "money shoots").TIME management on set IS a big deal! A good 1st AD can keep a show going, have a designated still photographer and...don't forget to have some fun while you're there; it could help you accomplish more faster.

OK off the top of my head things you dont have that I cant live without:
1/ Monitor
2/ Shotgun mic
3/ CTO / CTB for lights, diffusion, practicals (usually I use non standard practical globes)
4/ grip bag (some must haves: grip clips, extension cords, power boards, tape then more tape, extra light stands, boom arm)
5/ mattebox with rotating filter stage and filters if not at least a 72mm polariser
6/ bunch of foam core sheets I can cut up as needed
7/ reflector

There are probably others people like to have as a minimum.

A dolly.

I mean you could technically go out and get a lot of "camera movement" gear like a crane, a steadicam, etc, but I find dolly moves can be intercut into your film more often than the others (less chance of it being "gimmicky") and if it's done well, adds tremendous production value to your film, working the audience almost on a subliminal level.

Big white posterboard, you can white balance with them, and you can use them to reflect light for a softer lighting effect.

Sorry add to that a soundblanket or two.
Wow! I didn't even know that something like a soundblanket exists. Now that I googled for the word I know what it is. But could somebody explain how to use a soundblanket? And how effective is it?
