I bought mine May 5th, so this is AWESOME!! I just called my dealer, and they have the forms and I can't wait.

which dealer? I called B&H they dont have anything yet...

is the form generic or tied specific to you/your serial number/your dealer?

I called Adwar Video, and they got the forms...I don't know what to tell you, B&H should have them.

I'd urge caution on this. I'm one of the suckers who went in for their IDX promo. What a debaucle that's turned out to be. JVC's seems to have major logistical shortcomings, not to mention abhorrent quality control.

I have a good feeling that JVC went with Anton Bauer because IDX has taken so long to deliver the batteries to JVC for the Promo.
