Legal Documents for Scripts

Does anyone know where I can find legal documents for scripts?

I'm working on a screenplay based on a true story and have the permission of a living relative, but I'm looking for a legal document I can use for my records.

What makes you think you need one?

Thanks Shawn! I might see if legalzoom has the services to draft such a document. I couldn't find it in filmcontracts, but I did find some other forms I might need in the future...what a great resource!

Well, it's mainly for security purposes. I did read somewhere that you do not need permission to base a script on the story of a deceased person, but for this specific story, there are other parties interested in the rights to it. As I already have the permission from the relative, I just want something in writing.
