Invoicing for a Production Assistant?

Hi! Just finished my first PAID PA gig. Need to submit an invoice. Anyone care to share templates/ tips?

Congrats!

If you don't use an invoicing/accounting program, MS Word has a couple of usable templates. Or, if you're on a Mac and you have iWork, Pages has a couple good templates as well.

(replace everything within parenthesis)



INVOICE


(your name)
(your address)
(your city)

(your phone number)
(your email address)
SS: (xxx-xx-xxxx)


Invoice No: (random big number)
Date: (last day worked)

Project: (name of project)
Job: (for commercials, # can be found on callsheet usually, omit if wanted. not relevant to features/episodic tv)

To: (client name or production company)
(client address)



Description:
(X) Days (job description) @$(xxx)/day



Total Due: $(xxx)
