Image distortion w/ Redrock

I'm using a DVX100 with a Redrock Adapter and I'm getting a distortion in the sides of the frame that are sort of like an opposite fisheye effect. Here's a shot of what I'm talking about. Look at the right edge of the poster's frame. The lens is a nikon 135mm, and the settings on the DVX are Z81 and MF45.

Has anybody else encountered this problem? Any suggestions? Please help!

I'm guessing that maybe the camera isn't lined up parallel with the ground glass, but I'm not sure. Check to make sure it is, and that might solve the problem.
