I need some help

Tried this in another section first which looks like no one ever goes.

I know if I use someone elses music in my documentary, I need to get a SYNC license signed. But what about in general? Is it that important that everyone on film signs a release?

Especially need direction and help for this:
What and where can I find some "canned" releases that will do the job, possibly with a little editing in Word?

Is there a book on this subject? Please do not suggest seeing a lawyer! I get physically ill, claustrphobic, and my stomach & mind need intense mental control to avoid the propensity to hurl violently, even if it's empty...

I keep these on-hand as reference:

Contracts for the Film and Television Industry. Mark Litwak. New York, Samuel French.

Entertainment Industry Contracts: Negotiating & Drafting Guide (Vol.1) Motion Pictures. Donald Farber, ed. Matthew Bender Publishing Company, 1992.

As far as canned releases that can be easily modified, browse around sites that feature contests and festivals. You can usually find release forms similar to what you need in those places and then modify from there. Location, music, and talent releases are plentiful.
