How do I properly black balance? - I'm getting noise in my blacks.

A few questions about black balancing:

1. How do I properly do it? I've been putting the lens cap on and then selecting auto black balance, but I'm still getting noise in anything black.

2. How I often should I do it?

lens stopped all the way down, cap the lens, 0db gain, auto black balance. any noise present from then on is simply your typical camera noise, if it seems to be more noisy than it should be in that sense, check your workflow/picture profile setup (particularly black level and black gamma)
