I'm no expert at all and haven't used those exact settings, but I did use Cine 4 with pro colour. I found that crushing the blacks was a good place to start. I didn't use a LUT. I only use Casey Wilson's LUTs for SLOG 2 shot stuff. But for cine 4, I think you should just be able to grade from scratch.
I assume that you are shooting low light/night time stuff if using cine 4?
Unfortunately there's no curve in LUTCalc for Cine4 but for Cine1 (HG4 or HG4609G33).
You need to build a temporay LUT like SL2 => HG4 and then use the LUTAnalyzer to have HG4 as a recording gamma. Then you can build a HG4/S-Gamut => HG4/709 LUT which should get you fairly close. Wouldn't use any other target gamma in that case though, since the input gamma is not matching properly! http://www.dvxuser.com/V6/showthread.php?343503-Info-About-Hypergamma-Levels/page2
Another option would be to linearize Cine4 by hand, apply the color matrix to Rec709 and then go to you preferred gamma. Can be done in Resolve for example.
