You can attach it to a number of cameras (per the Cavision website). See link the below to read more about the actual matte box. They show a number of cameras using the matte box: JVC HD100U, Sony FX1, and others.

4x4 Bellows Matte Box

I bought it for my DVX100a but never had an opportunity to use it. It is in good condition. I never used it in the field. I only setup it on my DVX to make sure I knew how to setup it up and operate it.

Yes, per Cavisions website, it will work on the HVX200. See links below:

Matte box configuration for HVX200
HVX200 Setup (minus pistol grips, shoulder pad and follow focus)

The model is the MB413B-2, becuase it only has two filter stages. One is stationary the other rotates.

Lente Loco Studios:
It does not come with side flaps.

Note:
This setup comes with the following pieces:
MB413B-2 (Matte box)
ARR1385 (rubber adapter ring)
AR85-72 (conical adapter ring)
MBF-5P (French flap)
RS-15IIM (rod support system)
Hard case

Thank you.
