Focus chart

Does anyone have the DVX focus chart? I remember seeing it on here a while ago, and after much searching and finding broken links, I can't seem to find it. Could someone maybe email me the file? I have heard of some people making a small one and laminating it and putting it on their camera with velcro, could anyone send me one formatted to do that?

Re: Focus chart

So, using these numbers anywhere in the zoom range should be accurate?

In my broadcast upbringing, we always zoomed in to the long stop and focused up then pulled back to our desired FL. Follow focus was done without assistants and on-the-fly.

Now on film, we just measured from the focal plane (usually at the gate) and set our follow focus marks appropriately to correlate to our moves and blocking. So the chart above would be a FL independent "gate" to subject measurement regardless of distance to subject?
