filling out fs100/fmu128gb rebate form-anything i need to know?

ended up buying a fs100 kit + fmu128gb from the good folks at able cine/nyc.

filling out the rebate form this morning before the dec 15th deadline.

anyone who has done the rebate form, any potholes i need to be aware of?

anyone get their rebate check yet?

Just make sure you include the cardboard cut outs, the online form and a copy of receipt. The offer ends Dec 15, but you have until Jan 15 to send it...

cardboard cut-outs just from the fs100 carton or include the upc from the fmu128gb carton also?

I just bought both of these things too. I should receive my packages this Friday, but the invoice should read 12/12/2011 so I think I am safe. I was wondering how long it took for people to get their refund check after sending the rebate materials in.
 
For anyone who knows, which one is the actual UPC code that needs to be sent in. I see a barcode that is printed on the cardboard box. I see three barcodes that are stickers on the box. One starts with a P, the other with a S01, and the third barcode reads POS BAR CODE. Am I suppose to send all of these barcodes in?
