DVX setup sheet?

[yzmotoxer807]

I was posting some of the forms that I use on my website and thought that others might find this one useful. I use it to keep track of how I set the camera up from scene to scene (or for an entire production).

Best,

Matt

Okay really old post but...does anyone know of one of these setup sheets for the DVX100 or more specifically DVX100B (not really much of a difference so...)? Does anyone know of one or do I have to make my own?
