Today on a shot the middle two bulbs went off during a shot. I couldn't put them back on (tried it several times). After waiting 15 minutes I tried a again and they worked again for another 15 minutes until they failed again. What could cause the problem? (The bulbs are obvisiously okay.)

First guess is the electronics that control the voltage are defective. That or the bulbs may be on their last legs... You might try a hair drier on the balast box to get the electronics 'warm' quicker to see if they fail more quickly to verify that it is the electronics.
If this is new equipment, see if there's a manufacturer's warranty for this sort of failure.
The bulbs lifetime should not be reached by far. Where is this balast-box located? This is a new unit. Haven't used it before this short.
