checking focus

To check focus, we usually zoom in all the way, focus and zoom out to desired shot size. I was using a production house's DVX100 and realise that the focus is slightly off during extreme wide shot. (viewed the footage on big pro monitor)

What could be wrong?

For that shot, I'm using F2.8 and was about 3meter away from the subject.

I wasn't using any additional lens.

Is there a way to rectify it ourselves? Is there anything to adjust just like the big brothers has (betacam etc)
