I have a Bogen 316 Cine Head designed for use with cameras up to 35lbs. I've owned it for eight years. It has cosmetic damage like scratches and duct tape attached to it but it still works. Comes with quick release sliding plate. Here's some info direct from Bogen:

I hate to do this but it's going to be a best offer scenario. My on-line research hasn't provided me with enough information to determine fair, reselling price. If find some time, I'll do a little more digging and see if I can come up with something. In the meantime, I took some pictures:
