Anybody else have trouble with the Anamorphic Adapter mounting?

I can't seem to get the anamorphic adapter to cinch onto the lens. The adapter won't go flush. This doesn't effect the shooting, but occasionally the lens adapter will fall off my camera and stop my heart. Any advice about mounting the lens would be appreciated. I'm using it with my Glidecam, but I afraid to use the adapter on a car mount.
