An Invoice?

I have to email an invoice to a producer for whom I was a utility this weekend. I've never done this before. How would I go about this? Just name ,date, address,ssn, hours worked? any samples or templates out there? just don't want to look like a fool...

do you have microsoft office? it comes with a lot of templates that you can modify to your style and needs, thats what we use, no ssn, if you are doing business as they are going to ask you for a w9 to which you must put you ssn or your tax ID (this is texas by the way) do you have a production company name? and yes you have to detail everything you did, hours worked, rate, date, extra fees etc.

It is simple. There is no magic formula.

They just need your name, your social security #, the position you filled, the rate of pay and the dates you worked.

It is also good to include your address so they know where to send your money.
