using a rope attached to the top handle and walking the camera into the shot with the rope guiding the movement

dolly shot
the wheel chair dolly, and what is the best method ie the camera shoulder mounted, on a tripod with someone steadying it, ect

boom shots
i dont have any idea other than using my arm

Cant see the camera behaving - Be spinning and swinging - Not good at all IMO

Has been done before - Maybe a pillow in the lap with cam might help.

For a steady cam I would recommend checking out http://www.cs.cmu.edu/~johnny/steadycam/

I built one (as has another friend) and we have both used it with great results. It costs $15 for all the parts and assembles in 5 minutes. You can use various size weights at the bottom (I use 2.5lbs if it's going to be a long day of constant shooting - or 5lbs for a really nice shot).

It's beyond simple but really does help eliminate a lot of shake - and for $15 you can't beat it!
