Where to get a microphone holder adapter and those 4 screws?

I just called Panasonic support, and THEY don't know what parts I'm talking about. It's the little bit that fits between the DVX100b handle and their microphone adapter. It requires 2 6mm screws and 2 12mm screws. I somehow managed to misplace them, but now I need to mount a Rode NTG2, and this is what I need.

HELP!!!
 
rycote has a hot shoe extension or you can buy this http://www.bhphotovideo.com/bnh/controller/home?O=productlist&A=details&Q=&sku=133276&is=REG& addedTroughType=search and what i did was cut the bottom off to where all i had was the straight bar with the 2 shoes at the top and at the bottom drilled 2 holes to match up with mic holder on the cam and instead of using the orginal mic holder that cames with the cam i used that instead or you could buy this http://www.bhphotovideo.com/bnh/controller/home?O=productlist&A=details&Q=&sku=114982&is=REG& addedTroughType=search
