What was wrong with my Lens and 35mm Adapter

The ground glass must be skewed kinda giving it the tilt shift lens effects..

you need to make sure the GG is level, and do a backfocus check
 
It could also be something got onto the lense itself. Possibly some kind of oil. At 1:24 with the couple kissing it appears there with the back of guys hood is already fuzzy. Then as you cut to them facing the camera buildings on the right looks smudged. Same goes for the other people pics except at 1:30 with the two guys in white jackets, its gone. So if you were shooting all day with the same setup without any changes then I believe you just need to make sure you check the lens for any finger smears. That what it looks like to me.

For Mbala:

Because the NS10's is a studio standard in every major studio for reference checking. Prodipe is not.
