What do I need to get these shots?

I'm thinking some of you have seen Rich Lee's video:

http://www.rich-lee.com/BCR.htm

About halfway through there is some really cool shots that look like the camera is attached to the musicians, first low on their body and sometimes high up above them.
Does anyone know what equipment I need to get these shots or what I can build to achieve these shots? Thanks!
Mike

My uninformed opinion:

The range from the subject on those shots is amazing, and yet the movement is so responsive. Looks like the widest shots are faked in PP, but I'm still impressed.

For face-shots you might be able to pull it off with a DIY-contraption, but shots like that...look above...

Not faked

Rich took out the storri contraption in post; hence the magic
