What do crew sign?

Hi

I have releases for actors, art, location, etc. But for a little film a friend shot some of the video. What should I get from him, a release, a contract saying he was paid for his services, what?

Are there samples of what I need somewhere?

You need a "work made for hire" contract.

Yes, if you don't have a signed agreement/contract the person who shot your footage has no obligation to provide it to you. There is nothing that says you own the footage and you have no recourse if the two of you have a falling out and he destroys the footage. Always have all releases and contracts signed before you start shooting. Lesson learned.
