What camera/lens' are being used in this video?

Depending on your available lenses either use a longer focal length (85mm-105mm-135mm are all nice), or zoom in.
Open up the iris but only as far as is still sharp...unless you have a really good lens f1.4 will be soft, f2-2.8 will be sharper, a cheap lens may even need f4.
The number and shape of blades in your iris will contribute to the character of the bokeh (blur).
If neccessary compensate for exposure with ND. Lighting will play a part in separating the subject from the background.
With a small sensor you may also benefit from getting closer to your subject.

Two camera set-up, one camera farther away with a long lens on it for the tight shot with the very blurred background. Background looks like it could be 30-100 feet behind the subject being interviewed.

Not a fan of interviews where the subject is talking to an invisible interviewer that is far off camera axis. ( it's like they are talking to somebody else in the room that the audience will never see )

I prefer to have the interviewer ( usually myself, though sometimes it's my client ) stand close to or behind the camera, so the person being interviewed looks like they are talking directly to the audience. Far more intimate and impactful visually than having them talk to "outer-space".
