Hi,

Not sure if this is the right spot, feel free to move it.

Just wondering if anyone has a volunteer document they have used for people helping out on a film. I am getting volunteer insurance but need a document to have everyone sign.

I searched on the web and didn't find much. Mainly festival stuff.

I am sure once I get the insurance, they might have the form but I am sort of a planner.

Here's one you could make up yourself:


Volunteer Release

I, ___________________ who am over the age of 18 agree to work on a volunteer basis on the film project ___________________, expecting no payment. I give permission for my image to be used in the production and agree not to sue the pants off the producers if I am injured.

Here's what it would look like after your attorney works on it a bit:



Agreement to voluntarily work on a film production and release of the rights to my image produced forthwith

I hereby express my written and undersigned agreement to refrain from any liability cause of action pursuant to Civil Practice and remedies code section 14,22,and 34G (Incorporating 18IR 1662.G). I affirm that I am over the age of eighteen (twenty one in states of MI,FL,OR,MO,GH,andPL. )Should I decide to break this agreement and bring a lawsuit, I agree to retain a licensed member of the bar at a rate to exceed $650 an hour. I agree to give my firstborn and secondborn children to the producers to use as they see fit, and will never again text while driving.
