Varizoom Iris Control

I am using the Varizoom Focus/Zoom/Iris Controller with the HVX-200 and cannot seem to get the Iris to work manually, everything else is in perfect working order, only the iris is always in automatic mode no matter what I press, adjust, beg, plead, cry, kick, or scream. I did not see this in another thread (sorry if it is). Thanks for any help.

Did you go into menu #4 settings and make sure it is set to manual iris?
Make sure the switch on the left side of the camera is set to manual
Make sure the plug from the controller is seated in the camera?
Make sure the iris switch on the varizoom is set to "M"
