Using proxy files

Because of the limitations of my hardware, I am forced to both use CS4 AND downsample my original high res files to something smaller my machine can handle as proxies.

1-What is a recommended size and codec to use for proxies?

2-Unclear as to what stage you need to swap in your high res files. Is it immediately AFTER your editing but before any color correction- grading, OR can you wait just until before your final render?

You can use any format really (320x200 web or NTSC DV). Just make sure that the file names of your proxy files and your real source files are identical.

I'd substitute in your high res files after cutting but before doing any kind of CC or transitions.

I'd recommend upgrading your hardware. PP is not the best at proxy workflows.
