Unauthorized Documentary

Does anyone have any information on what and how an unauthorized documentary is produced. Are the subjects informed? Etc etc. Im looking on producing a music documentary.

It depends and varies from project to project. If your subject doesn't want the doc made, it's probably not a good idea to tell them about it.

If it's a music documentary, you also have to be concerned with legal issues revolving around use of their music.

Is there a reason why the documentary is "unauthorized"?

I'll go one step beyond and say unless the subjects are famous and/or otherwise 'in the public eye' I'd say you'd have to be worried about privacy rights and getting releases/consent forms. You can't just video someone and then use their image willy-nilly for your commercial endeavor. I mean, look at the show COPS for example. The alleged perpetrators have to sign releases giving the show permission to use their likeness otherwise they get blurred out.
