In my super-low-budget days I had the unpleasant experience of shooting a 16mm film for a director who almost put an actor's eye out.

There's a story about Star Trek's George Takei getting a little carried away with a rapier on set when an episode called for him to brandish one after the entire crew gets mentally incapacitated.
No one got hurt but if I recall other cast and crew were getting a bit concerned.
