Shot/scene Breakdown Sheet?, How should I organize it?

Does anyone know where I can find a template or some guide as to how I can organize my shot/scene breakdown sheet? I am really pulling my hair out trying to organize this thing! I tried to make a table, but it doesn't really work packing all of that information on there. Any suggestions would be greatly appreciated.

I usually make my shot list the morning of, and I simply go: "What scenes am I shooting today, what shots do I want in each scene?"

I then make a really simple list stating the scene, and the shots I need going from wide shots gradually moving to close-ups. Don't forget to think about the beginnings and ends of scenes as I've seen many people have entire projects that look like a string of vignettes because there's no coverage to transition us between one scene and the next.

Maybe I have a really naive outlook at shotlists but what information are you trying to cram into your tables?

I had a book that devoted a chapter to this. They start with the story board. From that they make a shot list, from that they make their call sheets, props, sets, locations, lights, camera, lenses, crew. There are filmmakers out there who know exactly what they're going to need and how long it will take. It's not that easy in real life as anyone who's made anything motion picture wise knows.

If you google "shot list" +template you will have examples of what other people use. Make your own or pick one that works for you.
