set safety?

I'm putting together a general "how to" for clients (corporate) about on-set safety. Bascially I've established a general caviate that members of the production crew are responsible for safety (grips/gravity, lighting/electrical, etc.) and are in charge of set operations to ensure safety.

However, I'm trying to find a good set of general safety rules for on the set, which I can share with clients, cast, crew, etc. for each shoot.

Does anyone have any suggestions for where I can find such a list? I know it's going to vary from set to set, shoot to shoot. But I'm looking for the basics. (If you see something unsafe, report it to the ____) If you see this, do not touch it. Never walk here... etc. etc. etc.

Thanks for any help

I am too in the process of hashing out a complete list of safety procedures. I am outlining safety for the crew, safety for the actors, and safety for the equipment.

For the actors, the biggest issue is lighting for me. I once had an actress burn herself severely "warming up" next to a hot lamp.

For equipment, I've always stressed that the safety of the equipment is crucial since much of it is rental.

For the crew, had some problems with electricity. Someone once dropped a field monitor, the glass shattered and was quite a mess. Another situation involved a dolly crash. Can't remember exactly how it happenend, but long story short, a dolly with camera operator and camera assist on it crashed into the boom operator.

In terms of set safety, i've had my share of fire issues and electricity consumption. I now install the rule that for ever two 500 watt lamp used, a fire extinguisher mush be within 10 feet.

Lighting is very concerning. For the majority of times, you will be shooting on location; often you are not aware of the location's power config. We were once shooting in the basement of an old house and had combined lighting wattage of 2500 watts. We blew fuses like crazy.
