Script Breakdown Forms For Word

Does anybody know of any free script breakdown forms that can be used with Word or Pages?

I'd rather type all the information in, but can't find any forms for Word. All I can find is .pdf files.

I really don't want to write everything in, but I'm afraid I may have to.

I'm actually using one as we speak (or type) and you can find it by searching Screenplay, Microsoft in google. Its the one with the microsoft link.

I created the Script Breakdown template in Word. It was way easier than what I thought it would be. I figured it would be a pain. But it was smooth.

I created a breakdown sheet in HTML. Since my day job is in web design, it's easier for me to do this kind of stuff at work if I'm in a web design application. Mine is layed out specifically for my needs for my current production, but perhaps you'll find it useful.
