My mistake.
Much is being made of this elsewhere so I think it bears addressing. The safety concerns were absolutely brought up to production by the camera dept. head, but the reasons for the camera crew departing was as much about late payment and housing disputes as safety. The reason I think this is an important distinction is that if the department's walkout was purely centered around safety concerns, that would cast Halyna's choice to continue working in a particularly negative light. I experienced a version of this myself on a low budget feature that I shot 20 years ago, so I have sympathy for the moral dilemma involved.
Sarah Zachary prop master said:"I went over to the cart because I had a suspicion of what happened, um and or, not what happened, but like you know to see if there were any other live rounds in the box that she pulled from. I started shaking the rounds and I found a few more in it," Zachary said.

The dolly grip (mistaken called a camera operator by the defence?) gives a pretty good run down of the normal protocols at around 5:49:00

.
the prop master has control over non firing props (dummy rounds) and the armorer has control over things that go bang,.

Ive been thining on your comments. For sure 'we' have a good concept of how a set should safely work from 'cart/safe to actor via armourer' which will keep the actor and camera crew alive. always.
I do see that the wider location needs appropriate treatment to control for example
-hobby shooting on days off
-robbery of any guns/ammo including props
-control of type of ammo ect.
Without those measure people could get hurt during breaks, when fooling around etc.
You do have some valid points there.
