release forms?

[Lake Films]

Hey. Anyone have a basic actor release form that you can send me, I lost mine and I have a rehersal tomorrow and want to get the 'legal' stuff done early. Or a place on the net? Thanks!

If so, please e-mail to me

This is for a non-compensated release. If you are paying the actor(s), you may need to change some words or find a diffrent form.
